RN/Nurse Practitioner HBPC
The Nurse Practitioner (NP) is responsible for the primary care of clients seen in the Home Based Primary Care program.
The NP is responsible for interviewing, evaluating, assessing, diagnosing and treating ambulatory patients presenting to the Primary Care Clinics.
She/he utilizes preventative clinical guidelines, appropriate nursing and medicine standards of care, and collaboration with team physician in providing care to patients with acute and chronic conditions.
The Nurse Practitioner conducts a comprehensive assessment of patients' medical, physical and bio-psychosocial status including:
Obtains a relevant health and medical historyPerforms a physical examination based on age and historyConducts preventative screening procedures based on age and historyUpdated evaluation of health status to determine need for further health careFormulates appropriate medical diagnosis based on data obtainedOrders appropriate diagnostic procedures and laboratory testsIdentifies and prescribes appropriate pharmacological and non-pharmacological agentsIdentifies needs of the individual and family as a result of the evaluation of collected dataProvides relevant patient/family educationConsults with collaborating health care professionals when doubt exists as to the diagnosis when expected improvement is not soon apparent and when specialized therapeutic or diagnostic techniques are indicatedDetermines the effectiveness of the plan of care through documentation of client care outcomesReassesses and modifies the plan as necessary to achieve medical and health goalsUtilizes Telehealth programs in providing care coordination throughout the continuum of carePerforms any procedures for which she/he has been credentialed This announcement is not intended to cover all aspects of the duties, responsibilities or qualifications of this position.
