Job Details
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this role, you will provide direct and individualized nursing care to patients based on the application of scientific nursing principles. This is a full-time, permanent position, offering an opportunity to work in a diverse range of medical settings, including home health, long-term care, and rehabilitation. The ideal candidate will have a minimum of 2 years of experience, with a strong understanding of medical terminology and a passion for patient care.
Responsibilities:
1. Provide high-quality patient care under the supervision of a registered nurse or physician, adhering to the standards and regulations of our healthcare facility.
2. Conduct patient assessments, monitor vital signs, and administer prescribed medications accurately and efficiently.
3. Assist in the planning, implementation, and evaluation of patient care plans.
4. Provide wound care and implement other specialized nursing care techniques.
5. Deliver care for patients in various stages of recovery, from pediatric to geriatric, and in various healthcare settings such as home care, long-term care, and rehabilitation.
6. Educate patients and their families on disease management, therapeutic procedures, and other aspects of post-treatment care.
7. Collaborate with the healthcare team to ensure the delivery of patient-centered care.
8. Maintain patient confidentiality and uphold the ethical standards of the nursing profession.
Qualifications:
1. Current and valid LPN license in the state of practice.
2. Minimum of 2 years of experience as an LPN, preferably in a home health, long-term care, or rehabilitation setting.
3. Proficiency in medical terminology and medication administration.
4. Experience in patient assessment, vital signs measurement, and wound care.
5. Certification in Pediatric Advanced Life Support (PALS) is highly desirable.
6. Demonstrated ability to provide home care and understand the unique needs of long-term care patients.
7. Strong interpersonal skills and the ability to communicate effectively with patients, families, and healthcare professionals.
8. Ability to work independently, manage time effectively, and make sound decisions.
9. Physical stamina to perform the duties of the role, including lifting and moving patients as needed.
10. Commitment to ongoing professional development and learning.
